What is Myomectomy?

A myomectomy is a surgical procedure used to remove uterine fibroids while preserving the uterus. This allows women to relieve symptoms such as heavy bleeding, pelvic pain, and pressure without undergoing a hysterectomy.

Fibroids are noncancerous growths in the uterus that can vary in size and number. Depending on their size, location, and symptoms, different surgical approaches can be used. While open abdominal myomectomy is still widely performed, Dr. Ezwana Elias specializes in minimally invasive techniques, such as laparoscopic and hysteroscopic myomectomy, offering faster recovery, less pain, and minimal scarring.

Types of Myomectomy

The best surgical approach depends on the size, number, and location of fibroids, as well as future pregnancy plans.

1️⃣ Hysteroscopic Myomectomy (For Fibroids Inside the Uterus)

This procedure is best for fibroids located inside the uterine cavity (submucosal fibroids). A thin, flexible hysteroscope is inserted through the vagina into the uterus, allowing the surgeon to remove fibroids without any incisions.

No external cuts or scars
Same-day procedure with quick recovery
Best for women looking to conceive

2️⃣ Open Abdominal Myomectomy (For Large or Multiple Fibroids)

In this traditional approach, a large incision is made in the abdomen to remove fibroids. While effective, this method has a longer recovery time and is only recommended when minimally invasive surgery is not an option.

Best for very large or deeply embedded fibroids
Requires a hospital stay of 4-6 days
Recovery time: 4-6 weeks

3️⃣ Our Specialty – Laparoscopic (Keyhole) Myomectomy

Dr. Ezwana Elias specializes in laparoscopic myomectomy, a minimally invasive alternative to open surgery. Instead of making a large incision, the surgeon creates a few small cuts (0.5 – 1.5 cm) and uses advanced laparoscopic instruments to remove fibroids with high precision. The uterus is then carefully reconstructed using fine suturing techniques.

Less pain and faster recovery
Minimal scarring and lower risk of infection
Hospital stay: 1-3 days
Return to normal activities in 2-3 weeks

Comparison Laparoscopic Myomectomy Open Abdominal Myomectomy
Scar Size Small (0.5 – 1.5 cm incisions) Large (8 – 10 cm incision)
Pain Medication Minimal More required
Hospital Stay 1 – 3 days 4 – 6 days
Time Off Work 2 – 3 weeks 4 – 6 weeks
Infection Risk Lower Higher

Who is a Good Candidate for Myomectomy?

Myomectomy is ideal for women who:

  • Have fibroid-related symptoms such as heavy bleeding, pelvic pain, or pressure.
  • Want to preserve their uterus for future pregnancy.
  • Prefer an alternative to hysterectomy.

For women who no longer plan to have children, a hysterectomy may be a more permanent solution, especially if fibroids are large, numerous, or recurrent.

Special Considerations

While myomectomy is effective, there are important factors to consider:

🔹 Bleeding During Surgery – Fibroids are connected to blood vessels, and removing them may cause more bleeding than a hysterectomy. Your surgeon will take precautions to minimize blood loss.
🔹 Risk of Fibroid Regrowth – If the uterus remains, new fibroids can develop over time, especially in younger women.
🔹 Precaution During Pregnancy – Women who conceive after myomectomy may be advised to have a Cesarean section to prevent uterine rupture during labor.
